Description
POSITION SUMMARY 
Provides active outreach to the community serving youth in Clay County, Missouri, to create partnerships to initiate and provide suicide awareness trainings. Provides outreach and case management to identified individuals needing mental health services.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
• Provide crisis intervention to youth experiencing a psychiatric or substance related crisis within the community. 
• Coordinate with schools and community regarding truancy, IEP, 504 meetings for Clay county crisis youth. 
• Facilitation of Psychosocial Rehabilitation (PSR) group.
• Provide transitional case management to Clay County crisis clients
• Monitor Clay County Crisis ECP and crisis monitoring
• Complete progress notes in AVATAR within 72 hours of service provided which clearly reflect interventions provided and necessity of those interventions. 
• Problem solve issues related to client’s safety, access to resources, basic needs, access to health care and mental health treatment.
• Maintain knowledge of agency technology programs and/or systems needed to do the job and use them effectively. 
• Maintain required training and certifications. 
• Maintain required agency productivity standards. 
• Attend and participate in weekly treatment team meetings, bi-monthly provider team meetings, and quarterly staff meetings. 
• Other duties and/or responsibilities may be assigned by supervisor. 


MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
  Education/Certifications:        
• Bachelor’s Degree in a related field required
• Valid Driver’s License and state-mandated auto insurance
• CPR/First Aid Certification required

Experience Required:              
• 1 to 3 years’ experience in a related field required

Required Knowledge:              
• Knowledge of youth behavioral health and related symptoms.  
• Knowledge of community resources and crisis management. 
• Strong understanding of Trauma Informed Care.

Skills/Abilities:                          
• The ability to build rapport with youth and their parents developing emotional regulation and identification of mental health symptoms.    
• The ability to create community relationships independently and to create and grow community connections. 
• Working knowledge of Microsoft Office programs, including Microsoft Outlook, and the ability to use the internet for basic tasks. 
• Ability to use basic business equipment.

The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this position include close vision, distance vision, and the ability to adjust focus. The noise level in the work environment is usually low to moderate.
